Nunkuwarrin Yunti is looking for a dedicated Case Manager, Harm Minimisation to provide culturally safe, client-centered support, develop tailored care plans, and connect clients with essential services and community networks. Job Description

About the Role

You will:

  • Undertake development, procurement and distribution of educational and promotional materials.
  • Provide inclusive, client driven case management for people with substance abuse issues.
  • Undertake, broker and/or facilitate access to support networks, aftercare and self-help groups and community activities, and assist clients to build/access these networks.
  • Liaise, advocate and broker services between clients and internal and external stakeholders as necessary.
  • Establish and maintain collaborative and productive working relationships with current and potential stakeholders and partners, both internally and externally.
  • Liaise and consult with other services to develop and support the provision of recovery and harm reduction orientated treatment that aligns with the client’s individual treatment plan.

 

About You 

You bring:

  • Minimum qualification is Certificate IV in Alcohol and Other Drugs, Aboriginal Primary Health Care, community/social health services or related field with minimum two (2) years demonstrated vocational experience in the related field consistent with position role and responsibilities
  • Experience in providing effective and well-informed advice, assessment, care coordination, advocacy, liaison and referral to community services
  • Well developed written and verbal communication skills and the ability to communicate effectively with Aboriginal and Torres Strait Islander people
  • Ability to liaise, network and negotiate with arrange of human services organisations and government agencies experience in the Northern and Southern Suburbs will be highly valued
  • Ability to deal with people with high and complex needs and conflict constructively
  • An understanding and knowledge of social, health and cultural issues effecting the Aboriginal and Torres Strait Islander population in Adelaide and the greater metropolitan region
  • Demonstrated experience in case management, care coordination, program coordination and delivery
  • Understanding of legal issues and legislation related to role.
  • Strong understanding of principles and delivery of comprehensive services to complex clients such as social and emotional wellbeing and group work
  • Highly developed knowledge of theory and practice in a range of interventions
  • High level negotiation skills with ability to resolve conflict successfully
  • Experience in managing a case load with high and complex need clients
  • Ability to research, analyse and contribute to project planning, development and delivery
  • Demonstrated high level communication, presentation and negotiation skills with people from a diverse range of backgrounds, professions and organisations
  • Ability to work with Aboriginal and Torres Strait Islander people and/or communities and their leaders, respecting traditional culture, values and ways of doing business.

Desired Skills and Experience

Desirable:

  • Previous experience working in community primary health projects/programs that demonstrate best practice for Aboriginal and Torres Strait Islander people and/or community.
  • Strong commitment to harm minimization principles and understand addictive behaviours, in particular people who inject drugs.
